About Us
The Nathan Foundation, founded in 2015 by Sonia Ekema, is dedicated to raising public health awareness and supporting families affected by Social Determinants of Health. Sonia’s journey began with the joy of motherhood and the challenges of her son Nathan’s Sickle Cell Disease diagnosis. After relocating to the U.S. for his care and later sending him back to Cameroon, she faced the devastating loss of Nathan in 2013. Through faith and resilience, she channeled her grief into creating the Nathan Foundation, turning her pain into a mission to help others. Her story inspires hope and demonstrates how adversity can be transformed into meaningful action.
Our team is comprised of passionate and dedicated individuals who are committed to making a difference. Everyone at Nathan Foundation shares a common goal of helping those in need, from our volunteers to our staff members.
Nathan Foundation employs a Community Health Worker (CHW) model to assist families facing social challenges that affects the health of individuals and the family. The CHWs works closely with families to identify challenges and connect them with resources, services, and support networks. This holistic approach deals with the root cause of health disparities, creating a resilient community.

We assess needs to collect comprehensive information about a family’s situation to design a personalized care plan that targets the root causes of health disparities and empowers families to improve their well-being. This is done in a conversational, empathetic and culturally sensitive manner.
This service focuses on providing families with donated household goods, including furniture, kitchenware, bedding, and essential household items. It supports families who may be transitioning out of homelessness, facing financial hardship, or starting over after difficult circumstances such as domestic violence or natural disasters.
Referrals are made to get expert help in addressing a social challenge. We have developed partnerships with institutions that run programs and services in our community which target specific problems such as mental health assistance, housing repairs programs, faith based organizations etc
