About Us
When Nathan was diagnosed with Sickle Cell Disease, Sonia faced a pivotal moment that ignited her fierce dedication to ensuring her son received the best possible care. This immense responsibility weighed heavily on her, yet she drew strength from her family’s steadfast love and support through thick and thin.
Relocating to the United States with Nathan was an incredibly tough decision for Sonia, especially as it meant leaving her 6-month-old daughter in Cameroon with her parents. The journey brought a flurry of emotions as Sonia faced various ups and downs with Nathan, ultimately leading to the heart-wrenching decision to send him back to Cameroon for better care.
On June 3rd, 2013, the Ekema family was struck by unimaginable tragedy with the loss of their beloved son…..
In those dark hours of grief, Sonia found strength in her faith and the unwavering support of her family and church family. Sonia navigated the depths of sorrow, discovering comfort and solace in God as she confronted her profound loss. Through this experience, Sonia’s resilience shone through, reminding her of the power of love and unity in times of unimaginable hardship.
Rather than succumbing to despair, Sonia transformed her pain into purpose which birthed Nathan Foundation in 2015.

The CHW Assessment Model has been design to target the five major Social Determinants of Health (SDOH). This model has questions and strategies to assess needs, strengths and barriers of families.
The community health worker model allows for the identification and immediate referral of families.
